Santa Clara Family Health Plan
Presentation of findings from an in-depth community assessment. Data sources included a community-led and collected survey, 20+ partner-organizations 1:1s, and system convenings. Assessment purpose was to inform the SCFHP investment strategy in the Southern part of Santa Clara County.

Report assessed community perspectives about barriers to equitable healthcare access in the Santa Clara County health & hospital system, and provided analysis and related policy recommendations. Primary data sources included 130+ patients and advocates.

This report & toolkit documents the process of an 18-month, multi-stakeholder, community-centered process to launch a Community center that was OF the community, not just FOR the community. Lessons learned, and the framework developed are also included.
